1. 申请者请务必随本表提交以下材料：Applicants of study need submit following documents with the application form:

最高学历的学历证书及公证件复印件；Copies of highest diploma, notarized document of highest diploma;
成绩单及其公证件的复印件；Copies of transcripts, notarized document of transcripts;
护照的复印件；Copies of passport;
体检表的复印件；Copies of Physical Examination Record for Foreigners;
无犯罪记录证明的复印件；Copies of Non-criminal Record;
证件照电子照片；Digital ID photo
推荐信／Two letters of recommendation (original)

*以上材料均须为中文或英文。无论申请人是否被录取，上述申请材料恕不退还。
All documents above should be in (be translated into) Chinese or English. Whether the candidate is accepted or not, all the application material will not be returned.

2. 申请非学历者需随表提交以下材料：Applicants of non-diploma study need submit following documents with the application form:

护照的复印件；Copies of passport;
体检表的复印件；Copies of Physical Examination Record for Foreigners;
无犯罪记录证明的复印件；Copies of non-criminal Record.
证件照电子照片；Digital ID photo;
动机信 Letter of motivation.

3. 不完整的申请表或未按要求提供有关材料，将会延误对该申请的审理时间；
An incomplete application form, or the supporting documents not submitted, WILL DELAY the process of your application.

4. 学历奖学金生必须住校。
Diploma students obtained scholarship should live on campus.

上述各项中所填写的内容和提供的材料真实无误；
All information and materials given in the form are true and correct.
在华期间，遵守中国的法律和法规，不从事任何危害中国社会秩序的，于本人来华学习身份不相符合的活动；
During my stay at LVTC, I shall abide by the laws and decrees of the Chinese government, and will not participate in any activities which are deemed to be adverse to the social order in China and are inappropriate to the capacity as a student;
在学期间，遵守学校的校纪校规，尊重学校的教学安排；
During my study at LVTC, I shall observe the rules and regulations of the university, and will concentrate on my studies and researches, and will follow the teaching programs made by the College.
在学期间，如出现严重身体疾病或心理疾病，或者日常生活行为严重违反学校纪律，对他人造成较大影响的，学校有权对本人采取必要行为，包括退学及取消签证。
During my study at LVTC, if I suffer from serious physical or mental problem, or if my daily activities seriously violate the discipline of university and have a greater impact on other students, the university has the right to take necessary actions, including dropping me out of school and canceling visa.
